The program officially launched on July 1, 2026, and is currently scheduled to continue through December 31, 2027.
The program is designed to bridge the gap between the growing demand for obesity treatments and Medicare's historically limited coverage of anti-obesity medications.
Which GLP-1 Drugs Are Covered Under the Medicare GLP-1 Bridge Program?
The Medicare GLP-1 Bridge Program covers a select group of FDA-approved GLP-1s. Not every GLP-1 medication or formulation is covered.
As of the program's launch, CMS includes the following medications:
Foundayo (orforglipron): Oral tablet.
Wegovy (semaglutide): Both injection pens, including WegovyHD, and oral tablets.
Zepbound (tirzepatide): Only the KwikPen.
What drugs are not covered under the Medicare GLP-1 Bridge program?
Not all GLP-1s are eligible for the GLP-1 Bridge program. The following medications and formulations are not included:
Zepbound single-use vials or single-dose pens.
Ozempic (semaglutide), which is primarily indicated and covered under Part D for Type 2 diabetes management.
Any GLP-1 medications prescribed for indications already covered under standard Medicare Part D benefits, like Type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic metabolic dysfunction-associated steatohepatitis/MASH. In these cases, patients should use their regular Part D plan rather than the Bridge.
Off-label uses or compounded medications.
Can Any Medicare Beneficiary Participate?
Not every Medicare beneficiary is eligible for the Medicare GLP-1 Bridge Program. To participate, you must be prescribed a covered GLP-1 medication to reduce excess body weight and maintain weight loss. You must also meet the Medicare GLP-1 Bridge Program's clinical eligibility criteria and obtain approval through the required prior authorization process before coverage can begin.
You must also be enrolled in an eligible Medicare Part D prescription drug plan. The program is available to beneficiaries enrolled in standalone Medicare Part D Prescription Drug Plans (PDPs) and Medicare Advantage plans that include prescription drug coverage.
Beneficiaries enrolled in Special Needs Plans (SNPs), Employer/Union Group Waiver Plans (EGWPs), and the Limited Income Newly Eligible Transition (LI NET) program are also eligible to participate. People who qualify for both Medicare and Medicaid may participate if they are enrolled in an eligible Part D plan and meet the program's clinical requirements.
If you have other Medicare plan types, such as a Program of All-Inclusive Care for the Elderly (PACE), you may not be eligible for the Medicare GLP-1 Bridge Program unless you are also enrolled in a standalone Part D Prescription Drug Plan.
Who Is Eligible for the Medicare GLP-1 Bridge Program?
To be eligible, you must meet all the medical and lifestyle criteria of the program. All qualifying participants must be a minimum of 18 years old, prescribed a GLP-1 for reducing excess body weight and maintaining weight loss, and commit to lifestyle modifications. This includes diet and exercise that align with the FDA-approved label of the medication.
Your prescriber’s prior authorization document must also verify that you fulfill one of the following clinical criteria:
Have a BMI of 35 or higher, or
Have a BMI of 30 or higher with a diagnosis of one or more of these conditions: uncontrolled hypertension, diastolic heart failure, or chronic kidney disease Stage 3a or higher.
Have a BMI of 27 or higher with either pre-diabetes, symptomatic peripheral artery disease, or a previous myocardial infarction or stroke.
Who Is Not Eligible?
You may not qualify for the Bridge Program if your GLP-1 medication is already covered through Medicare Part D for another FDA-approved use.
The Bridge Program does not apply if you are taking a GLP-1 medication to treat Type 2 diabetes, moderate to severe obstructive sleep apnea, or metabolic dysfunction-associated steatohepatitis (MASH), even if you meet the Medicare GLP-1 Bridge clinical criteria.
You should continue to receive coverage through the standard Medicare Part D prescription drug benefit rather than through the Bridge Program. The Medicare GLP-1 Bridge is designed to increase access to GLP-1s for people who seek the drug solely to reduce excess body weight or maintain weight reduction.

Schedule an evaluation with a healthcare provider
The first step is to meet with a healthcare provider experienced in obesity medicine and GLP-1 therapies. If you meet the eligibility criteria, your healthcare provider will prescribe a GLP-1 medication covered under the Bridge Program.
A prior authorization is submitted
Before Medicare will cover the medication, your provider must submit a prior authorization form through the Medicare GLP-1 Bridge Program.
Medicare reviews the request
Once the prior authorization has been submitted, CMS reviews the documentation to determine whether you qualify for the Medicare GLP-1 Bridge Program.
If additional information is needed, your healthcare provider may be asked to submit further documentation. Once the request is approved, you will also receive an email from Medicare confirming that your medicine is covered.
Fill your prescription
Fill your prescription at your pharmacy and pay $50 for a 28- or 30-day supply. You will not need a new approval from Medicare to get a refill as long as you stay on the same drug, even if your dose changes.
